The state of Iowa requires all 9th grade students to have a dental screening and dental screening form on file. If a screening form has not yet been submitted to school, dental screenings will be done free of charge and done automatically by Dental Hygienist Shannon McManus, from the Iowa Dept. of Public Health on Tuesday, Jan. 12 and Friday, Jan. 22.  Please send proof of any dental exam within the last year, or complete the attached consent form located at https://5il.co/od4v .  Parents will be notified of any dental concerns.  If you have any questions, please contact the school nurse.
